It's been a decade since the teenager was diagnosed with the rare genetic condition Chromosome 8, Monosomy 8p - which causes developmental delays that lead to intellectual disabilities and physical abnormalities.
'Having a special needs child in a lot of ways gets more challenging with me worrying more about her future the older she gets,' the 55-year-old alum wrote to her 1.8 million Instagram followers.
'My mom always had said, "A special needs child is an angel from God and Eloise is the biggest angel with the biggest selfless heart." I'm so proud of her strength and kindness.

'I'm so grateful for her older sisters, my friends who are my family and my dad who is the best grandpa [and] father figure for Eloise. [He's] always there for her, no matter what. We [love] you girlie! Happy birthday, Weezy.'
